Summer physicals for middle, high school sports 8/15 & 8/27

• Sports physicals for C.J. Hooker Middle School students will take place from 8 a.m. to 1 p.m. on August 27 at the the middle school nurse’s office.

• Sports physicals for Goshen High School students will take place from 8 a.m. to 1 p.m. on August 15 at the high school nurse’s office. 

Prior to having a physical at school, please do the following:

1. Complete the permission form for a school physical.
2. Sign up for a physical by emailing CJH Nurse Dawn Pawliczak (grades 7-8) or GHS Nurse Nancy Ellefsen (grades 9-12).
3. Register for sports on Arbiter Sports (formerly Family ID).
   • JV/Varsity registration opens July 27.
   • Modified registration opens Aug. 5.
