Return to School/Work Form for Students and Employees

The Return to School/Work Form for Students and Employees must be completed and submitted to the school nurse or main office in order to return to school or work after:

  • Receiving a positive diagnostic test result of COVID-19;
  • After an exposure to an individual with COVID-19/mandatory quarantine;
  • After being sent home for displaying symptoms of COVID-19.

In addition to the form, students and employees must fulfill the following criteria in order to return:

Requirements to return to school/work

If you had a positive COVID-19 test result, you may return if:

  1. It is at least 10 days since you received your positive COVID-19 test result.
  2. You do not display any of the following symptoms:
    • A temperature greater than or equal to 100 degrees (without the use of fever reducing medication)
    • Feel feverish or have chills
    • Cough or sore throat
    • Loss of taste or smell
    • Fatigue/feeling of tiredness
    • Shortness of breath or difficulty breathing
    • Nausea, vomiting or diarrhea
    • Muscle pain, body aches or headaches
    • Nasal congestion/runny nose
    • Unexplained rash
      • If you believe you have any of the above symptoms because of another diagnosis, such as allergies, you must confirm this with your healthcare provider NOT the school nurse.
  3. You return the completed Return to School/Work Form for Students and Employees (at right).

If you had mandatory quarantine due to exposure, you may return if:

  1. It is at least 10 days since the date of exposure.
  2. During the quarantine period, you did not display any of the following symptoms:
    • A temperature greater than or equal to 100 degrees (without the use of fever reducing medication)
    • Feel feverish or have chills
    • Cough or sore throat
    • Loss of taste or smell
    • Fatigue/feeling of tiredness
    • Shortness of breath or difficulty breathing
    • Nausea, vomiting or diarrhea
    • Muscle pain, body aches or headaches
    • Nasal congestion/runny nose
    • Unexplained rash
      • If you did display any of the above symptoms during your quarantine period, you must contact your healthcare provider for assessment.
      • If you believe you have any of the above symptoms because of another diagnosis, such as allergies, you must confirm this with your healthcare provider NOT the school nurse.
  3. You return the completed Return to School/Work Form for Students and Employees (at right).

If you were sent home from school/work for displaying symptoms of COVID-19, you may return if:

  1. You have a note from your healthcare provider.
    • A COVID-19 test may be issued by your HCP but is not mandatory to return.
  2. You return the completed Return to School/Work Form for Students and Employees (at right).
