By New York State Law, all students in grades K, 1, 3, 5, 7, 9, and 11 and all new entrants into the school district MUST have a recent PHYSICAL EXAMINATION done by their doctor or by the nurses’ office. This is due within the first 30 days of the start of school.
New students are also required to show proof of immunizations prior to entering school. As of September 2016 meningococcal immunization is required for students entering grades 7 and 12.
NY State Department of Health Immunization Requirements
If you do not provide proof of immunization during registration, the school nurse will be in contact with you shortly after your child enters school. There is a FREE immunization clinic available to all residents. Click here for information about clinics run by the Orange County Health Department. Appointments are required.
The NY Department of Health Immunization Requirements Form provides full details of immunization schedules and doses necessary for school attendance.
Additional information about immunization requirements is available through the NY Department of Health.
- All entering 6th graders require a total of two varicella/chicken pox immunizations or proof of immunity and one TDaP (tetanus, diphtheria and pertussis) immunization.
- Beginning Sept. 1, 2016, students entering 7th and 12th grades must be vaccinated against meningococcal disease in order to attend school in New York state.
NYS Required Screening
DUE TO THE COVID-19 PANDEMIC, THESE SCREENINGS WILL NOT BE REQUIRED FOR THE 2020-21 SCHOOL YEAR AS PER THE STATE OF NY.
Students are screened in school for vision, hearing and scoliosis according to New York State mandates. The state mandates are:
- Vision (distance and near) – all new entrants and in grades Kindergarten, 1, 3, 5, 7 and 11.
- Hearing – all new entrants and in grades Kindergarten, 1, 3, 5, 7 and 11. 3.

- Boys in grade 9
We make every attempt to keep students in class for instruction time. Scoliosis screening is performed during Physical Education classes.
Parents/guardians will be notified in writing of results of all screening examinations that may require further follow up with your student’s primary medical provider.
Please contact the school nurse’s office or family medical practitioner with questions or concerns.
