Notice about body mass index reporting for New York State

New York State Education Law requires collection of body mass index (BMI) data on your child’s physical. Physicals are required by NYS for grades K, 1, 3, 5, 7, 9, 11 and new entrants.

Each year, the State compiles BMI data from a sample of school districts. This year, Goshen will be a part of that survey and will be reporting to New York State Department of Health information about our students’ weight status groups.

Only summary information is sent. No names or information about individual students are sent. However, you may choose to have your child’s information excluded from this survey report.

A form was emailed to all parents on December 12. You can also get a form from your child’s school. If you do NOT wish to have your child’s weight status group information included as part of the Health Department’s survey this year, please fill out the form and return it to your child’s school nurse.
