Please note – there is no paper form this year. Paper forms will NOT be accepted.

The Goshen Central School District is proud to communicate its students’ academic, extra-curricular and athletic achievements, as well as classroom activities and school events, to the local community. Goshen students are often featured on the district website, social media (Facebook, Twitter, Instagram, YouTube), printed and electronic newsletters, and press releases to local newspapers.

Your child’s name, photograph and information may be shared to showcase his or her achievements and/or participation in school events/activities.

IF YOU DO NOT WANT YOUR CHILD’S NAME/INFO SHARED VIA PUBLIC RELATIONS, PLEASE COMPLETE THIS FORM. This does not include honor roll lists or yearbooks.
